Our practice is committed to caring for you during this unprecedented time. As we adapt and respond to COVID-19, we are taking steps to provide an even safer environment within our practice.  We will continue to care for patients with all concerns including in-clinic procedures, referrals, and any urgent needs. Nurse triage will also still be available to answer questions.
 
In order to reduce the risk of exposure to COVID-19 while also preserving quality care, we have implemented the following changes:

  • We have adjusted our approach to collaborative care. You may see a different physician or nurse practitioner than you were originally scheduled to see, but rest assured that you will still receive excellent care.
  • Telehealth visits (see section below) is now available.  You may see one of our physicians or nurse practitioners remotely via video using your mobile device or computer.
  • We have begun screening patients at the check-in.  Patients with concerning respiratory symptoms will be asked to reschedule and guidance on home care will be provided.
  • In an effort to reduce waiting room time and crowding, we ask that all sick patients remain in your vehicle while you call us to “check-in”.  You will then be notified via your cell phone when the medical assistant is ready to bring you back for your appointment.

Telehealth Information:

Use a computer or device (cell phone) with a good internet connection and webcam.

We have established a structure for telehealth services:

  1. Download ZOOM for your iPhone or Android phone.
  2. If using your computer, go to: https://www.zoom.us/
  3. Each provider has a dedicated link to join their “meeting” that will be sent to you by email or text.  Also, you will be given a numerical password.
  4. Log on 5 minutes before your scheduled appointment time to make sure everything is working properly.  You will be sent to a virtual “waiting room”, where the provider will invite you in when it is time.
  5. Allow ZOOM to access your device’s microphone and camera (if you are joining by video).

Advanced Care Planning

  • Getting information on the types of life-sustaining treatments that are available.
  • Deciding what types of treatment you would or would not want should you be diagnosed with a life-limiting illness.
  • Sharing your personal values with your loved ones.
  • Completing Advance Directives to put into writing what types of treatment you would or would not want – and who you chose to speak for you – should you be unable to speak for yourself.
